An unusual cause of A2 pulley rupture in a 49-year-old man.

چکیده
A 49-year-old man presented to his family physician with pain in the third finger that occurred after he tried to grasp a falling heavy object. His finger was forcefully extended from a semi-flexed position. On clinical examination extension was normal. However, flexion was slightly limited and extremely painful at the end point of flexion. Ultrasound of the finger was performed and showed that the flexor tendons at the level of the proximal phalanx were at an abnormally increased distance from the bone (Fig. A, B). Both the superficial and deep flexor were also markedly thickened. The normal slips at the side of the tendon representing the A2 pulley were replaced by a heterogeneous mainly hypoechoic area. Fluid was seen deep to the tendons in between the latter and the bone. MR imaging was performed using a dedicated wrist coil on 3 T (Fig. C-D). On transverse and sagittal proton density weighted images the flexor tendons were seen to be located at some distance from the bone at the midlevel of the proximal phalanx. The ulnar attachment of the A2 pulley could not be visualized, and the radial attachment was thickened, elongated and exhibited high signal intensity on the transverse images. Fluid sensitive sequences showed fluid in the synovial sheath and also in between the tendons and the bone.

A New Method for Removal of Supraglottic Leech Infestation, an Unusual Cause of Hemoptysis.
Foreign bodies in upper airway may have various presentations and be life threatening. Leeches can attach to upper airway and cause serious problems. Herein we report a 55-year-old man with hemoptysis due to attachment of leech and explain our technique for its removal.
متن کاملAbdominal Splenosis: An Unusual Cause of Bowel Obstruction in a 7-Year-Old Child
Splenosis is a rare condition defined as a heterotopic auto-transplantation of splenic tissue, typically after blunt abdominal trauma. It can occur anywhere in the abdominal cavity, or even the chest. Splenosis found in clinical practice is relatively rare because most patients are asymptomatic. We report a rare case of abdominal splenosis in a 7-year-old boy, 3 years after an abdominal blunt t...
متن کاملSubclinical Partial Attritional Rupture of the Flexor Digitorum Profundus as an Etiology of Atraumatic Trigger Finger
BACKGROUND Trigger finger is a relatively common clinical entity. The etiology is most often attributable to stenosing tenosynovitis though traumatic cases have been described. When trigger finger is associated with an underlying flexor tendon rupture, previous cases have reported a clear association with overt laceration or previous trauma. METHODS We present the case of a 23-year-old male a...
متن کاملReverse Trigger Finger: Does it exist? A Case Report
Trigger finger is a common condition, occurs primarily at the A1 pulley and tenosynovitis is the usual cause. However, although rare, triggering of the flexor tendon can occur in other areas. We present previously unreported condition in adult: triggering at the distal end of A2 pulley, which was caused by nodular thickening of the flexor digitorium profondus tendon with degeneration. A 79-year...
متن کاملImpact of taping after finger flexor tendon pulley ruptures in rock climbers.
Flexor tendon pulley ruptures are the most common injury in rock climbers. Therapeutic standards usually include a prolonged use of taping applied as a replacement for the lost pulley in a circular fashion at the base of the proximal phalanx. Our biomechanical considerations, however, suggest a new taping method, the H-tape. The purpose of the study is to evaluate whether this new taping method...
